National Association of Professional Geriatric Care Managers - Professional Geriatric Care Managers (PGCMs) are health and human services specialists who help families care for older relatives, while encouraging as much independence as possible.  Use this site to learn more about the services of Geriatric Care Managers or to find a Geriatric Care Manager in your area.

Who Qualifies for Home Care?

  • Anyone who was recently released from the hospital, but not fully recovered after a surgical procedure or cardiovascular disease.
  • Anyone who is living with a chronic condition such as diabetes, kidney disease, or stroke that requires frequent monitoring.
  • Anyone diagnosed with a new condition or placed on new medications.
  • Anyone who has a terminal illness and needs personal care and pain management.
  • Anyone with mobility issues who needs assistance with activities of daily living or requires physical therapy.
  • Anyone may request CARE Home Health Services for themselves or a loved one.
  • CARE Home Health Services will file all claims and handle all paperwork.
  • Services are provided under Medicare Part A and B and Medicaid.
  • More insurance companies are providing home health benefits; please check your policy.
  • CARE Home Health Services is available 24 hours a day, 7 days a week.
