- Anyone who was recently released from the hospital, but not fully recovered after a surgical procedure or cardiovascular disease.
- Anyone who is living with a chronic condition such as diabetes, kidney disease, or stroke that requires frequent monitoring.
- Anyone diagnosed with a new condition or placed on new medications.
- Anyone who has a terminal illness and needs personal care and pain management.
- Anyone with mobility issues who needs assistance with activities of daily living or requires physical therapy.
- Anyone may request CARE Home Health Services for themselves or a loved one.
- CARE Home Health Services will file all claims and handle all paperwork.
- Services are provided under Medicare Part A and B and Medicaid.
- More insurance companies are providing home health benefits; please check your policy.
- CARE Home Health Services is available 24 hours a day, 7 days a week.
